New Patients
 | A new patient can expect to spend 1 to 1 and 1/2 hours at the office for their first visit. This will include enough time to fill out the necessary entrance forms, have your consultation, examination, and first adjustment. If you have your entrance forms completed already, you can deduct 15 to 20 minutes from that estimate. The 2nd visit will take approximately 30 minutes as you will presented with a complete report of findings. Subsequent visits are typically 15 minutes or less. |

ACN / United Health Care Form: If United Health Care is your insurance carrier, you must fill out this form in addition to our Entrance Forms. ACN Group, who manages your visits for United Health Care, uses this form to determine the number of visits you will be granted through "managed care". Fill this form out as accurately and completely as possible. If you have had an accident or trauma that caused your current condition, be sure to write down the details.
